What you allow: I allow commercial use for most of my products providing they are not creating premade pages and selling them.  All requests are handled individually.  I also require them to fill out a commercial use request form detailing what they intend to do and with which products.  If they purchase more products and wish to use them in the same manner, they must fill out another form.
Fee: Generally no additional fee, though this may change eventually.
